The Short-Term Rehabilitation program is for people who need to recover from stroke, heart disease, or other debilitating conditions. Participants are expected to return home with the skills and confidence they need to live independently after a hospitalization.
The person-centered program is designed to be tailored to each individual. Short-Term Rehabilitation provides physical, occupational, and speech/language therapy. It also offers a wide range of interprofessional support, including nurses, doctors, therapists, and social workers. We provide services to help you improve your strength, independence, and quality of life, including Therapy and medically integrated programs.
Only at American Senior Communities, Road to Recovery allows our multidisciplinary team to meet with the patient, their family, and the patient within 72 hours of admission to help map their Road to Recovery. This includes identifying discharge goals.
It is always a desire to return home as soon as possible. Rehospitalization can be a possibility if seniors are not properly rehabilitated. Our team will perform an in-home safety assessment to ensure patients can safely navigate their homes.

Speech/Language Therapy aims to improve functional communication and cognitive skills and to teach safe swallowing through diet modifications and other feeding techniques. Therapy can include compensatory strategies, muscle retraining, and communication devices.
We track senior rehabilitation outcomes and use that data to improve our services.
